Grade Reassessment form: Faculty of Science

Prior to completing this form, please review the detailed information about the final grade reassessment/appeal process.

Complete this form if you are requesting a grade reassessment for a Faculty of Science course (including AS, BI, CH, CLIM, CP, DATA, ES, GESC, GG, GL, HE, HN, KP, MA, PC, PS, SC, ST, UU150, WASC). This form is to be used for Step 2 of the process (where the student remains dissatisfied with the response of the course instructor).  The form will be received by the Chair of the department offering the course.  This form should be submitted by students when they can demonstrate that the grading criteria for an evaluation were not applied correctly or there was an error in the calculation of their final grade.

The reassessment process is limited to concerns regarding the application of grading criteria to the work submitted and/or the calculation of grades only.

Therefore, the following issues cannot be addressed through the grade reassessment process:
  • Procedural matters regarding the application of classroom, department or university policies (e.g. the application of late policies, changes to the syllabus, etc.)
  • Poor performance due to personal extenuating circumstances (e.g. sickness, family emergencies, etc)
  • Dissatisfaction with the course structure, grading scheme, number of assignments, etc.
  • The instructor's personality, demeanour, and/or attitude.

These types of concerns should be directed to the faculty petitions committee or the department chair/program coordinator, as appropriate.

Grade reassessments must be submitted no later than six weeks following the formal release of grades from Enrolment Services.

If you wish to have multiple evaluations re-assessed, please submit one form per evaluation.  Each request should contain its own unique explanation of why the evaluation merits a grade higher than the one assigned.  Do not copy and paste the same information across multiple requests.

Before submitting this form, students are required to attempt to resolve their concerns with their course instructor.  Please submit below a well-organized Word or PDF document that includes verbatim copies of any correspondence with your instructor regarding your grade.  If you discussed this matter with your instructor verbally, please provide a summary of what was discussed.  These details will be confirmed with your course instructor.

If you have attempted to contact your instructor, but not yet received a response, please allow one week for them to respond before submitting this form.  After this time, if you have received no response from your instructor, please submit a copy (in Word or PDF format) of any correspondence you sent in your attempts to contact them.
Attach a Word or PDF file that provides a detailed explanation of how the evaluation meets the assessment requirements at a level that exceeds the mark given.  Specific reference to the contents of the student's written work and its relation to the evaluation objectives/grading criteria is needed.  Submissions that are vague or simply state that they think their work merits a higher grade will be rejected without consideration.

Remember that the reassessment of your grade will only take into account the work you submitted, the evaluation objectives and the grading criteria given by the instructor (if any were given).  As such, statements regarding the following will be disregarded:
  • the account of effort put into the evaluation
  • your grades in other classes or on other evaluation
  • needing a certain grade to stay in a program, maintain a scholarship, get into graduate school, etc.
Attach a file that provides information regarding the evaluation's objectives and grading criteria/rubric.
Provide a copy of the evaluation submitted, including any feedback received.
